Fever in Babies and Toddlers: A Parent's Quick Reference
Every parent knows the 2 a.m. forehead check. This is the calm, practical reference for what to do next — written to be skimmed one-handed while holding a warm little person.
What counts as a fever
A fever is generally 38.0°C (100.4°F) or higher. Where you measure matters: rectal and ear readings run slightly higher than oral; underarm runs slightly lower. Always note the method along with the number.
Call or go in right away if…
- Baby is under 3 months with any fever — contact a healthcare provider promptly, even if they seem okay.
- Fever comes with a stiff neck, unusual drowsiness or difficulty waking, laboured breathing, persistent vomiting, signs of dehydration (no wet diapers), or a rash that doesn't fade when pressed.
- A febrile seizure occurs — stay calm, protect from injury, and seek care.
- Fever lasts more than 2–3 days, or keeps climbing past 40°C (104°F).

How to actually measure a squirmy child
- 0–2 years: rectal measurement with a digital thermometer is the gold standard; a no-touch forehead thermometer is excellent for frequent checks without waking them.

Comfort beats numbers
How your child looks and behaves matters more than the exact reading. Offer fluids often, dress them lightly, and use fever medication per package or provider directions for comfort — not to chase a “normal” number. A playful child at 38.5°C is usually less worrying than a listless one at 38.0°C.
